Assumption of Risk

I understand and acknowledge that participating in the World Championships involves inherent risks, including but not limited to serious physical injury, strain, or even death. I voluntarily assume full responsibility for any risk of injury or harm that may arise during or as a result of my participation in the event, including activities performed during both training & competition.

Release of Liability

I hereby release, discharge, and hold harmless the FTA or their agents, staff or volunteers, and affiliates from any and all claims, liabilities, demands, actions, or causes of action arising from any injury, illness, or damage that may result from my participation in the World Championships. This release includes, but is not limited to, injuries arising from negligence, accidents, equipment usage, or following staff instructions.*

Medical Disclosure

I confirm that I am in good health and both physically ad mentally capable of participating in the World Championships event. I agree I have disclosed any pre-existing injuries, medical conditions, or physical limitations prior to participating in the event. I further agree to inform the FTA of any changes to my health or physical condition that may affect my ability to safely participate either before or during the event.*

Emergency Medical Treatment

In the event of injury, accident, or illness during the World Championships, I agree to the FTA on-site staff to contact emergency medical services on my behalf if deemed necessary. I understand that I am financially responsible for any medical treatment provided and will not hold the FTA responsible for any injuries, as stated above.*

Media Release

I agree that that all training sessions and event competitions will be filmed and may be used for social media promotions, live streaming or promotional materials for the World Championships, including both video and photography and I give my permission to use my likeness in the above.

Release of Liability for Minors (Parent's to Sign)

I voluntarily agree to allow my child to participate in the event/activity and accept full responsibility for any injury, loss, or damage incurred as a result of their participation. I consent to emergency medical treatment for my child if deemed necessary by a qualified medical professional during their participation in the event/activity, and to all associated costs. I, the undersigned, am the parent or legal guardian of the above-named minor.*
